Business Fives Waiver of Liability and Release Form

By participating in the Business Fives football tournament held by Business Fives Ltd, I understand that participation in this sporting event is potentially dangerous and that physical injury, including serious injury may occur.

As team captain I agree that on behalf of my team, myself and all individual players will accept any liability for personal injury arising out of my teams participation in this event.  I also agree to disclose in advance any underlying health conditions for members of my team that the organiser should be aware of prior to taking part.

On behalf of all the players in my team I hereby release any claim of any nature which I or my team might have against Business Fives Ltd from any liability for any injury incurred during this event.

Please be aware that failure to complete this form will mean that the team and or specific player will not be permitted to play.

By completing this form on behalf of your team you have signalled your intent to sign this document and have expressly given your consent to sign this document electronically.

This waiver will be kept securely so it can be produced when required.  A copy of this waiver can be requested at any point in time and will be sent to the signer on request.
